Please read the following instructions to register yourself and schedule your appointment.

Simple as 1-2-3.

  1. Registration – complete personal and insurance information.
  2. Exam – complete the relevant assessment for your evaluation.
  3. Schedule – book your appointment with a therapist licensed in your state.

 

PLEASE COMPLETE ALL SECTIONS IN ONE SITTING.

Although some patients may finish sooner we recommend allowing at least 30-60 minutes to complete. We respect your privacy and will never disclose your personal information without your written consent.

We will check your insurance benefits for mental health coverage and find out if we can accept your insurance only after you complete your registration. We can determine your out-of-pocket expense, if any. There is no cost to complete the patient registration and patients only pay at the time of the clinical interview. 1. Registration

At the bottom of this page you will see a link to get started with patient registration. Fill in the demographics and insurance information and then submit. Make sure to fill in all required information as identified by an asterisk (*).

2. Assessment

 After you register you will be directed to an online assessment. Answer all questions as accurately as possible and make sure to answer all required questions before submitting. We recommend allocating one hour and doing the assessment in one sitting on a desktop or laptop computer.

3. Schedule

The last step is to schedule your clinical interview. Select the state in which you reside and you can search for available slots with the calendar. You will be matched with a therapist who is licensed in your state so that you can pick an available slot and confirm the appointment. Prior to your appointment, you will receive a reminder and instructions for your appointment. Please plan to be somewhere with adequate Internet connectivity (preferably Wi-Fi) and access to a phone, tablet, laptop, or desktop with a video camera enabled. Just prior to your appointment you will receive a secure link to connect with your assigned therapist via a HIPPA-compliant audio-video channel.
